چکیده

Abstract Objectives Long-term care facilities (LTCFs) may act as a reservoir of ESBL-producing Enterobacterales (ESBL-E) and carbapenemase-producing (CPE) for hospitals the general population. In this study, we estimated prevalence molecular epidemiology rectal carriage with ESBL-E CPE in residents Dutch LTCFs between March 2018 December 2018. Methods were geographically selected across country. For each LTCF, random sample tested To identify risk factors high and/or individual carriage, characteristics subset collected. WGS was conducted on isolates from an >10% all to institutional clonal transmission. Results A total 4420 159 included. The weighted mean 8.3% (95% CI: 6.8–10.0) no found. 53 (33%), where >10%, MLST using (wgMLST) performed. This included 264 isolates, majority being Escherichia coli (n = 224) followed by Klebsiella pneumoniae 30). Genetic clusters identified more than half (30/53; 57%) ESBL-positive LTCFs. Among E. blaCTX-M-15 (92/224; 41%) blaCTX-M-27 (40/224; 18%) most prevalent ESBL-encoding genes. K. common (23/30; 80%). Conclusions is resistance observed mainly predominance blaCTX-M-27. seems comparable previously reported

Current prevalence of multidrug-resistant organisms in long-term care facilities in the Rhine-Main district, Germany, 2013.

Multidrug-resistant organisms (MDRO) and in particular multidrug-resistant Gram-negative organisms (MRGN) are an increasing problem in hospital care. However, data on the current prevalence of MDRO in long-term care facilities (LTCFs) are rare. To assess carriage rates of MDRO in LTCF residents in the German Rhine-Main region, we performed a point prevalence survey in 2013. Swabs from nose, thr...

The Emerging Threat of Multidrug-Resistant Gram-Negative Organisms in Long-Term Care Facilities

BACKGROUND Infections caused by antimicrobial-resistant bacteria are associated with substantial morbidity and mortality. Residents of long-term care facilities (LTCF) are among the main reservoirs of antimicrobial-resistant bacteria, including methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE). Multidrug-resistant gram-negative organisms (MDRGN) are e...

Fluoroquinolone-resistant Escherichia coli Carriage in Long-Term Care Facility

We conducted a cross-sectional study to determine the prevalence of, and risk factors for, colonization with fluoroquinolone (FQ)-resistant Escherichia coli in residents in a long-term care facility. FQ-resistant E. coli were identified from rectal swabs for 25 (51%) of 49 participants at study entry. On multivariable analyses, prior FQ use was the only independent risk factor for FQ-resistant ...
